On the whole I use the book method, but sometimes in a spread I suddenly become aware of connections between cards, a pattern not obvious from the individual book interpretations. Then I just let this intuition guide me. The querent usually agrees that when I do this, the results are meaningful.

It's a bit like musical improvisation. I once heard the keyboard player Jools Holland say, "I always improvise, based on a lifetime of study and learning".
